Late at night Kazue drives alone on a mountain road when she hits something with her car. Stopping in a panic, she gets out to check what happened, and sees a naked girl with long hair falling over her face. The girl has a birthmark just below her left eye and looks familiar. Before Kazue can ask her name or if she is ok, the girl runs away. Kazue chases her through the woods, finally arriving at a mansion. Entering the mansion, she discovers a different girl wrapped in a sleeping bag and on the verge of death. Her name is Yukiko. Kazue rushes Yukiko to the clinic where she works.
One year later, Yukiko has been fully restored to health, but all recollection of her life before Kazue rescued her has vanished. There has been no report of a missing person matching her description, so she lives at the clinic. She and Kazue have become friends. Kazue continues to wonder about the naked girl that she encountered that night when Yukiko was rescued. She looked almost like Rei, the daughter of the clinic? head, Dr. Togashi, except for having a birthmark just under her left eye?Rei had disappeared a few days before that night.
Federal investigators arrive at the clinic and request an interview with Yukiko. Kazue deals with them, and is shown photos discovered in the nearby mountains. There is a picture that looks like Rei, but the investigators say it? Tomie. Investigator Yamazaki tells Kazue about the ?omie Incidents.?A murder always happens in any place that Tomie appears and the victim is always Tomie herself. If Tomie is killed her cells can regenerate and bring her back to life. She can also take over another person? body to bring herself back to life. At the time of Yukiko? disappearance there is a possibility that she might have met Tomie. Kazue refuses the investigators request to see Yukiko due to her condition. Investigator Yamazaki concedes, and mentions that he thinks the head doctor? daughter Rei might be Tomie.
Kazue is troubled by this last remark, and goes through some things found near Yukiko when she was first discovered, looking for clues, and finds a video tape. Kazue watches the tape, and sees a grotesque scene. The group of male college students who disappeared in the nearby mountains a year earlier, Yukiko wrapped in a sleeping bag, and a girl yelling ?urry! Kill her!?It is Rei? voice, but Tomie? face with the birthmark under the left eye!
A student who was part of the college group, Hamaguchi, breaks into the clinic, and kidnaps Yukiko, carrying her off to the mansion again where Tomie is waiting. Tomie feeds off the internal organs of one of the college students, a scene beyond imagination?
Dr. Togashi has suspected a link between the disappearance of his daughter Rei and the Tomie incidents, and shows up at the mansion carrying a shotgun. He pulls the trigger while saying to Tomie: ?ay you rest in peace? The bloody, dying Tomie draws near to Dr. Togashi with her final strength. Tomie?or is it Rei? The birthmark beneath her left eye disappears as she clings to her father and weeps.
The mansion is surrounded by patrol cars. Kazue once again rescues Yukiko. It seems Yukiko is safe again, but Tomie has other plans?
